Photography Permission

By signing this form and participating in classes you are giving permission and the rights for Dayne Williams to include you in pictures and videos (taken during classes and of your fight) for the promotion of

All pictures and videos will be used strictly for the promotion of and to document your progress throughout the program.

Pictures and videos may be used on the website, on promotional printed material such as flyers and posters or on social media platforms such as Facebook, YouTube and Instagram.

No pictures or videos will be taken during personal training sessions without verbal consent.

Cancellations and Refunds personal training sessions, classes and concession purchases are non refundable and expire 5 months from the date of purchase.

If a trainer has to cancel a class or personal training session for whatever reason without giving you more than 6 hours notice then you will not be charged and will be credited with a free session.

If you fail to attend your teams training sessions and start falling behind in the program you will be given the opportunity to attend the opposing teams training sessions or on the weekend to catch up.

Failing to catch up and progress may result in you being withdrawn from the program without a refund.

Intellectual Property and Copyright

All content supplied through and including email resources, restricted blog posts and other membership content remain the property of Dayne Williams and Boxing 101 New Zealand Limited.

By participating in or personal training with Dayne Williams you are permitted to use the copyright content for personal use only.

It is your responsibility to inform Dayne Williams or any trainer if the intensity of a training session or class is beyond your capability and we will adjust the session accordingly. You have the option of slowing the workout down by practicing technique or end the session in which case it is a cancellation on your part.

Ensure you are well hydrated, rested, have had a meal or filling snack within 3 hours of a training session and always bring a water bottle with you.

I the undersigned believe that to the best of my knowledge, all of the information supplied on this form is correct. I consent to taking part in this exercise program and I will not hold trainers nor Dayne Williams liable in any way for any injuries that may occur while I am training.

I have read, understand and agree to the above conditions which will apply to all classes and personal training sessions.
